Ssd Application

Posted on February 5, 2010.
Ssd ApplicationCan I, as a spouse, a sign of my husband DSS application? He has dementia. A form must read it said that a sign?

It is in a nursing home with dementia Frontal Lobe. I had him sign the facility as "close". I did the online application of SSD, and received copies for him to read and sign ... I'm not sure he can do ...

Get a proxy form and you can do it legally

Hastily he forges

If it is not competent, he can not validly sign any legal document for himself. In this case, it needs to have someone who has the legal authority to sign for him.

The easiest way to do it would be with an attorney, a legal document allowing another person specified in the proxy to act on his behalf. A proxy is one of several documents, and others as a will and an advance medical directive in which any lawyer Estate planning will include preparation of an estate plan for a client. If you and your husband has no estate planning, check and see if you do not.

If you do not, it may be too late to make a run, because it would be legally competent to sign it to be effective.

Instead, you need to ask a court to be appointed his legal guardian and / or restaurant (the words vary from one state to another). Talk to a lawyer because it sounds like there may have more questions you will need to deal with from that point.

You can take the form to him, I read it, then you read it, and have the sign!
