When to Apply

Once you become disabled, you must apply for disability benefits. Indeed, it may take some time to process an application for disability benefits, usually three to five months.

What to submit

To apply for disability benefits, you must complete an application for social security benefits and the relationship of people with disabilities.

In doing so, the following information must be submitted:

aec Social Security number;

aec birth or baptismal certificate;

aec The names, addresses and telephone numbers of doctors, health providers, hospitals and clinics and the dates of your balance sheets;

aec The name and quantity prescribed of all medications you take;

aec Medical records from your doctors, therapists, hospitals, clinics and social workers in your possession;

aec Laboratory and test results;

aec A summary of where you worked and the type of work you have, and

aec A copy of the latest W-2 form (Wage and Tax Statement) or a federal tax return for the year, if you're self-employed.

In addition to this form, other forms must be filled. A form for collecting information about your condition and how it affects your ability to work. Other forms give doctors, hospitals and other health professionals who have treated you permission to send information about your health.
